Forms
The following forms are required to be completed and returned to the school office before a child is permitted to participate in sports:

     Parent Participation Pledge
     Athlete's Emergency Information Form (Parent)
     Athletic Release Form (Physician & Parent)

The forms below are for coaches/assistants and any other parent volunteers to gain the required clearances prior to working with the children:
